About this Form

1. This form enables you to apply for membership of the Friends of Churchill Island Society or renew your membership of the Friends of Churchill Island Society.
2. You will be able to review the contents of your form and return to the form to make changes to your input before you press the Submit button.

Membership Rates

• FAMILY MEMBERSHIP: $33.00 per year
  A Family Membership applies to a household consisting of two or more adults without any children where at least one adult is not eligible for a concession, or one or more adults with one or more children.
• SINGLE ADULT MEMBERSHIP: $22.00 per year
  A Single Adult Membership applies to a household consisting of a single adult who is not eligible for any concessions.
• CONCESSION MEMBERSHIP: $17.00 per year
  A Concession Membership applies to a household consisting of one or more adults where all members of the household are eligible for a concession, i.e. Pensioners, Seniors Card holders, Full-time Students, and Apprentices.

Payement Options

You will be able to select one of these payment options:

1. Credit Card/Debit Card.
2. Apple Pay (Apple Device + Safari).
3. PayPal.
4. Funds Transfer using PayID.
5. Funds Transfer using BSB Number and Account Number.
6. Cheque.
7. Money Order.
